Once you become a patient of our affiliated professional practices for medical or mental health services, use the OurHealthPlus Pharmacy services, or use the services of any member of our Organized Health Care Arrangement, any Personal Information you have provided through your account while using the Services will become subject to these affiliates’ joint Notice of Privacy Practices.

Your Sponsor Organization or the subscriber of your benefits will not be provided with this information if you have already given them similar information in your employee file or record, unless you are a family member of an eligible employee.
